Fill in the application form<\/h4>\n<p>Please fill in the <a href=\"https:\/\/www.sgmo.ch\/qualitaet\/swiss-cancer-network\/zertifikat-swiss-cancer-network-scn\/\">electronic application form<\/a> (only available in German or French). In order for the certificate to be awarded, at least 80% of the quality criteria listed in the application form as well as the mandatory criteria must be met.<\/p>\n<\/div><\/section><br \/>\n<div  class='hr av-zdaet-e6026feece0ec8957a73885074582450 hr-default  avia-builder-el-4  el_after_av_textblock  el_before_av_textblock '><span class='hr-inner '><span class=\"hr-inner-style\"><\/span><\/span><\/div><br \/>\n<section  class='av_textblock_section av-lf80th64-17d1e426865e8f0572b40636e5c4b49c '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\" ><div class='avia_textblock'  itemprop=\"text\" ><h4>2. Submit additional documentation<\/h4>\n<p>After receipt of the application form, your application will be subject to a formal examination by the office. This takes up to five working days.<\/p>\n<p>You will then be asked to submit the following documents in support of the information provided in the application form:<\/p>\n<ol>\n<li>Organisation chart hospital\/practice<\/li>\n<li>Tumour board organisation chart incl. tumour board appointments in the current year<\/li>\n<li>List of responsible specialists according to the tumour board&#8217;s compilation with proof of their FMH title or equivalent<\/li>\n<li>Proof of external further training of nursing staff<\/li>\n<li>Anonymised tumour board report<\/li>\n<li>Care network organisation chart<\/li>\n<\/ol>\n<p>You can find more information in the separate <a href=\"#\">Guide<\/a>. Content review by the Certification Committee<\/h4>\n<p>Once the office has received the requested documents in full, your application will be forwarded to the Certification Committee (Dr. med. St\u00e9phane Cochet, Prof. Dr. med. Ursula Kapp, Dr. med. Walter Mingrone) for examination of the content. The examination by the Certification Committee usually takes up to 4 weeks.<\/p>\n<\/div><\/section><br \/>\n<div  class='hr av-81st-87b88fed060a0571f2d4ca6eae18567b hr-default  avia-builder-el-8  el_after_av_textblock  el_before_av_textblock '><span class='hr-inner '><span class=\"hr-inner-style\"><\/span><\/span><\/div><br \/>\n<section  class='av_textblock_section av-eo40t-0608cde9847d30ba8e6020d4fabe587e '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\" ><div class='avia_textblock'  itemprop=\"text\" ><h4>4. Award of the Certificate<\/h4>\n<p>If the evaluation by the Certification Committee is successful, the certification contract and the invoice for the annual fee will be sent to you. A sample contract can be requested from the office at any time. The certificate will be issued as soon as the countersigned contract has arrived at the office and the annual fee has been paid. The certification is <a title=\"Zertifizierte Institutionen\" href=\"https:\/\/www.sgmo.ch\/qualitaet\/zertifizierte-institutionen\/\">published<\/a> on the SSMO website.<\/p>\n<\/div><\/section><br \/>\n<div  class='hr av-8i1f9-31d6597618dcff3736343689e357caa2 hr-default  avia-builder-el-10  el_after_av_textblock  el_before_av_textblock '><span class='hr-inner '><span class=\"hr-inner-style\"><\/span><\/span><\/div><br \/>\n<section  class='av_textblock_section av-lf80v44b-2efbed5218b0c8451093a71e22338cf2 '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\" ><div class='avia_textblock'  itemprop=\"text\" ><h4>5. Annual renewal of the certificate<\/h4>\n<p>The certificate is valid for 1 year. You will automatically receive the necessary documents to apply for renewal 4 weeks before the expiry date (<a href=\"#\">Renew Certificate<\/a>) . The following quality criteria must be met at the latest at the first renewal of the certificate: A1; B1; B2; B4; B6; C12<\/p>\n<p>Not all documents have to be recreated for renewal of the certificate.
